Currently riding 20” Velare’s on low profiles with 40 mm lowered.
The ride is horrendous! Feel every lump and bump in the road.
Would 18” rims with deeper profile give me a smoother ride?

40mm lowered how? Just lowering springs on original shocks? If so I’d guess changing wheels will have very little impact.
The consensus seems to be that suspension has a much greater effect on comfort than an extra centimetre of sidewall.

I had the van lowered before delivery. I’m guessing it was just springs on original shocks. Totally clueless when I bought the van!
Would it be a case of replacing shocks as well to improve comfort?
Is it an expensive job to keep it lower with different shocks?
 
I disagree - large wheels and super low profile tyres do affect the ride as do the type of tyre. OEM shocks can also be improved upon.

I went in a van on 19s / solow’s and it felt like it didnt have any suspension. I didnt like the ride one bit.

I’ve ran a mix of 18s and 17s with -40mm springs and koni shocks and it rides perfectly. When fitting 17” winters with taller side walls it felt very slightly softer.

That said, aesthetics and ride are a very personal thing and what we all want / will accept is different, so feedback will vary accordingly.
 
Most of the comfort will come with a good suspension set-up. 40mm isn’t a big drop but on standard shocks will feel cr4p. Yes, smaller wheels with bigger side walls will help but you will notice the biggest difference by sorting the suspension.
What pressure are your tyres? That can make a difference too.
 
Change the wheels to more sensible proportions 17" or 18" fit decent tyres some tyre reviews may indicate what may be best. A good set of AS 3PM&S would give all round security and have a suspension review fit a set up that restores the height has good dampers KONI would be my choice and enjoy the ride. Is only a passing fashion fad having dropped suspension overly large wheels shod with elastic bands. Most of the time one cannot even see them as you will be driving it, unless you have a bench permanently parked to look at the things or a camera set up to watch them while going on your way. You may even enjoy driving again who knows :D
